Scheduling your initial appointment is easy.
You need three things to schedule your initial appointment:
~ A major credit card ~ Your contact information including a phone number and ~ Your email address
A $25 deposit charged to a major credit card is required to make an appointment. Upon check out, the $25 dollar deposit will be applied to the visit fee.

For patients who are NOT participating in one of our customized weight management programs, the following policies apply:
Follow up appointments for patients who are not participating in one of our customized weight management programs, require a $25 deposit.
To rescheduling your appointment, simply call us at least 24 hours in advance. Please make every attempt to reschedule your appointment 24 hours or more before the scheduled appointment time. Rescheduling an appointment less than 24 hours before the scheduled appointment time will result in forfeiture of the deposit in its entirety. You will be asked by our staff for an additional $25 deposit to hold the appointment for your next visit.
To cancel an appointment, please call us 24 hours in advance. No-shows or appointment cancellation less than 24 hours before the scheduled appointment time will result in forfeiture of the deposit in its entirety. Same day appointments and appointments made less that 24 hours before the scheduled time cannot be cancelled. Cancellation of such appointments will result in forfeiture of the deposit in its entirety.
